Have you ever been in a situation where the condom came off during intercourse, and didn't know it until it was almost to late?  If it's happened to you, what was your reaction?  Or how would you react?

How would I react? Well, a bit freaked.

One cannot cry over spilled milk, but just deal with the situaion at hand.
How 'intact' was it still? If it simply came off but stayed in place, and no cum went inside her, I think you're both ok. I have learned to use lube even when you think you don't need it, and when finished, pull out slowly, with the hand at the base of your penis to feel for the condom.
If it was not intact, and you came inside her, she can douche with a stronger mix of vinegar and water, or a very diluted blend of peroxide and water for her own quick comfort.
Take note of what might have caused it to come off. Brand? Size?
Put a little dab of lube on the tip of your penis before and after putting on the condom, and that usually helps it to 'adhere'.

When I was in college, a condom broke during sex with a friend.  I was really freaked out.  Much more than she was.  Birth control was my only concern at the time, and I guess from her calm reaction that the condom was redundant in that matter.

Still, I couldn't really relax for days.

Always (100%) use of condoms =
0.9 seroconversions (HIV) per 100 person-years

Never (0%) use of condoms =
6.7 seroconversions per 100 person-yrs

Overall an 85% reduction in risk of seroconversion (HIV) with condom use

Regarding gonorrhea transmission & condom use:

45-78% reduction in gonorrhea infection with 100% condom use (not adjusted for correct usage)

71% reduction in gonorrhea infection when condoms used always and correctly.

Condom breakage rate: 0.4-2.3%

From a related report from the NIH the THEORETICAL risk of HIV transmission with condom use(not based on clinical data) is as follows:

If failure to use condom = a relative risk of HIV tranmission of 1.0, then the following rates of transmission THEORETICALLY:

1) condom used - breaks = 0.006

2) condom used/no break but
fails visible water leak test = 0.000008

3) condom used/ no break/
no visible leak/virus still passes
through condom = 0.00000004
