What is the best way to find out a provider's rules when you are with her? (sometimes you can only find out so much from profiles and reviews) I want to be a gentleman and treat her with respect but also have the best possible experience. Is it best to ask her straight out at the beginning of the encounter or just see what the limits are as you go along?

you build up a rapport with her and she feels comfortable with you... just be polite and ask ....or ask her if you can ask a question or two ... I will ask her what is her favorite position or have you ever tried this, etc .... remember just be open, honest, and polite ....hobby safe, hobby well, hobby all you can be

My provider cam out and told me at the begining of our session that she doesnt kiss on the lips or do anal, so that put me at ease and I suggest a few postions and we tried them.
When I'm with a lady for the first time, I just ask after we start the foreplay. Honesty goes a long way.
what a client likes is to have like a 15 minute chat in the beginning...sometimes I cant keep my clothes on that long..but when I can I like to ask what their favorite positions are and what turns them on sexually. You might not get a great response from some providers but in general if you read reviews and be respectful like you said you should have no problems figuring out what they will and wont do. The best way is to ask upon arrival and when all have become comfortable.
I'd suggest asking her straight out - but not crudely. I'd also suggest waiting until you're both undressed or in intimate contact... sometimes explicit questions right off the bat make the ladies nervous.
