I am planning on meeting with an independent provider in Vegas who has 8-10 ratings on her looks and performance reviews, with great GFE juicy details for VIP's. Her rates are 400 an hour. For that rate, should I expect (within reason) the same experience that I read in the VIP sections, or should I plan on having a negotiation stash of cash nearby?  
If there are consistent 8-10 ratings, does that mean that my initial cash in the envelope, will provide me the service I expect? Or is it standard to expect to pay more once the door is shut. Of course I plan to tip another 20-50 if the service is right.

Occasionally some ladies expect a modestly extra amount for Greek but it would be spelled out as such on her website (or in her TER profile).

Tipping is never expected as well, but don't let me discourage you from doing so, as long as she exceeds your personal threshold of pleasure.

You should expect her regular menu and realize that 400 is a standard price for a good and well reviewed provider in Las Vegas.

If she is getting 8-10s in her reviews, she is not up selling.  Added charges for some adventures as mentioned about is normal, and a lot of the ladies won't offer them on the first visit anyway.

Tipping is good, re booking if she earns it is even better.
