It flairs up and eases for no apparent reason. It runs in his family but I really don't know if it's hereditary. Btw, he's a fit guy who eats healthy

Forgive this over simplification and be aware there are many approaches to acute and chronic gout.
The acute attacks occur when uric acid crystals cross the "lining" around a given joint. So either when uric acid levels are rising or falling the time is most ripe for an acute attack.  
In general, the acute attack can be relieved by the use of relatively high doses of prednisone or other steroid for a relatively short time.  Generally relief is relatively quick. (In my experience, within 24 to 36 hours.)
To prevent further attacks, there are a few meds that lower the uric acid. Your physician will pick the best one for you. He/she will also give you a very low dose of a very old medication (a form of colchicine in a very low dose) during the period when he/she is lowering your uric acid to prevent another acute attack.
It is important to control uric acid because uric acid can cause kidney stones, kidney damage, and joint damage.  
Physicians generally know how to handle the problems of gout

I'm sorry, I have a friend that suffers of the same condition but he's a totally sweetheart.

I believe I have the female version. A sharp numbing pain at the tip of my big toe underneath the nail. Woke me up out of my sleep the other night. Seems to come only when the weather Is cold.
