So I found a provider on P411 that looks like someone I would like to meet. No reviews, no website. Checked phone number and name in my state and her stated home state. No information found.
So my question, how thoroughly are providers vetted before being able to have an add on P411?
Would you roll the dice or is it a bit too risky?

As far as provider vetting to get on p411, they need to be invited by a client member in good standing who has 5 or more okays, or the provider needs recent reviews on a reputable site by established reviewers.
This provider may have had reviews and dropped them, or still has them but they are not linked to p411, or she got on by invitation. So if a provider is on p411 you can be relatively sure she is a legit provider, probably not LE, probably not a ripoff, but you have no idea of her level of service.
If you are a newbie you should stick to well reviewed providers (even if they are on p411) and leave the TOFTT for experienced mongers.

Iv'e been around for a while, but this is out of my comfort zone because of the lack of reviews. Too bad because she is hot.
Personally, I will 'occasionally" see a P 411 provider without any reviews at all, but if you have even the slightest doubts about doing so I agree with PS that you should simply wait a bit. If she is great today, she will still be great in a few days or weeks, and if she turns out to be TGTBT, then you saved yourself some aggravation and some money.
More and more, I see ladies delisting themselves for one reason or another. Some have added more details to their profile to give you an idea/perspective of themselves.
The OP doesn't give any idea of who the lady in question is so that he can get an honest opinion.
I've encountered this exact situation. No reviews, no website. Similar to you, I googled her phone number and/or e-mail address in hopes of finding something. Nada. So I just sent her a short, genuine PM or e-mail and asked her if she had any reviews. I've done that probably a dozen times. Every time the lady responded. Most of the time (~ 9/12) she either sent me a link to her reviews or provided one or more of her ID's/sites she was reviewed on. One time the lady was very honest and told me that she had reviews (3 total), but that they weren't good and she wanted to start over (her reviews weren't terrible --- she had an average > 8). Maybe twice the lady told me that she had a no review policy. One of the ladies that I met this way turned out to be an absolute gem. Just don't reach out to her if you aren't ready to pull the trigger.

from a fellow monger, then I'm most likely taking a pass. I am willing to wait a few weeks or months until she gets a few reviews to better base my decission.
