requesting white listing

I have several reviews on ter but i can't figure out how to request a listing from the provider.  Do I just request via an email after the event or is there a button I am overlooking?

A polite request in a thank you e-mail is all you need. The provider has to have the free limited Provider VIP membership in order to White List a client.

First, a provider has to whitelist you; so there is nothing for you to do. It's her validation that zyzy is okay to see in a session.

As someone who relies on his whitelist for screening, and likes to have current "okays",  I always ask at the end of an appointment if the lady is amenable to white listing.  I'll then send a follow up E-mail a day or two later, again thanking her for her time and if I've written a review I'll let her know its posted or forthcoming. If she hasn't gotten around to whitelisting me at that point, I'll politely ask again in that e-mail for her to do so. But that's it. If she doesn't whitelist me, so be it... She's moved on to future clients and I understand. I don't ask her again.

But also remember that one or two whitelists may mean nothing to some providers in the screening process--they still might want additional concrete references from providers you've seen. As you get more and more whitelists, it will definitely help facilitate the process.

or when she is leaving from seeing you for whitelist.Keep in mind not all providers whitelist some will and some only do it after seeing a guy more than once. But it doesn't hurt to ask.
Most of the whitelists I have given I just gave it the guy didn't ask but when guys do ask usually it is at the end of the appt or via email.
