Often this is done because the provider prefers either in or out. She makes you pay more for the out if she prefers in and vice versa. I can especially understand why a visitor to NYC might be more reluctant to take an OC appointment just because of the logistincs of it. Then again, maybe she just prefers incall.

Another price strategy you will often see is when a provider publishes a higher price for a one hour appointment because she prefers multi-hour appointments.
