Personally, I rarely have dates that long without a restaurant or bar being involved. However, when I have, snacks have been a must, as  is a good bottle of something or other, if you drink. If you're going to stay in the whole time, it becomes all about the chemistry--e.g., if you and she can't enjoy talking to one another for an extended time, it will get awkward real fast. Some of my favorite chats ever have been in bed, after the fact. Even the Energizer Bunny probably couldn't copulate for four hours straight.

Generally, you'd start with some conversation, move to the action, recharge with more talking, maybe some drinks or snacks, more action. Basically an ebb and flow between mental and physical stimulation. I've never done a 4 hour encounter, but I would probably start with drinks at a bar before moving onto the main course.

I've had a lot of 2 hour sessions that were just in the room, no problem with that as far as I've seen.

For a 4 hour session, usually involves dinner or bar or something more public / social.  One of my favorite dates in SoCal was a 6 hour date, walking along the beach, hanging out, lunch, back to the room, then a couple of hours there.

Many ladies offer a lower rate for a mix of time, 2 hrs in + 2 hrs out being less than 4 hrs in for example.  Some don't.  Everyone I've seen has, but that doesn't mean it's right or wrong just part of my selection process.

I've never gone longer than 2 hours as a first date, though.  As said above, 4 hours together (whether private or public) if you aren't hitting it off can be pretty awkward.
