Let's say a provider or her agency advertise "My fee is all inclusive of service and tipping is not required but appreciated." And the fee is $200/hour. If your were satisfied with the service, would you give more? Or do you consider the Ad that the tip is built in? And if you did tip, did you have regrets later that you tip too much or tip none at all?

Only in extraordinary circumstances. Providers can set their rates anywhere they want. Even those who work for agencies know what they are getting themselves into and have the option to set a higher rate rather than annoy their clients with upselling. If a lady wants to earn more, and the law of supply and demand will support a higher rate, she should set a higher rate rather than troll for tips. Just MHO.

To me, the fee is for some preconceived level of a pleasurable evening. If my evening was noticably better than I expected, I like to show it in some way. Occasionally a tip, but more often a gift selected specifically for her, based upon what I have learned about her. The message that, "I listened and took the time to get something specifically for you" is as important as the object itself in saying, "You were special".
No offense meant to the ladies here, but tipping just for the sake of tipping is really a waste of money. I only say this because the hourly rates charged are more than adequate compensation for a standard session. I know ladies incur expenses, the job has risks, etc.. but let's face it, the hourly rates of escorts are more than a Harvard trained Brain Surgeon or the President of the U.S. make per hour.
Now if the ladies goes above and beyond the call of duty, a tip is most likely warranted. Above and beyond the call of duty means different things to different people, it's your call to decide if she went out of her way to make your appt a good one for you and then to tip her accordingly.
