However, I think it is the responsibility of the client to ask for a reference.  I admit that once I said, "I've seen this lady".  The new lady called the lady I had seen and all turned out well.  However, I felt guilty for not asking for the reference.  

I say this is timely because a lady who I get along great with knew me only by my phone number, first name and TER handle.  When I wrote to her with my full first and last name, she did not recognize me.  I had the chance to refresh this ladies memory before the new lady called for a reference and everything went smoothly.  

As a lady, I would ask the man if he has got the green light to ask for a reference before calling another lady.  This helps avoid any confusion up front.  I know I won't refer a lady to another lady unless I ask again.

Ladies,
When you vouch for a client as a reference, you are essentially saying "this guy's OK. I would accept an appointment to see him MYSELF". There's a responsibility in that.
If you can't remember him, or you have any comments about discrepancies or are in ANY way UNSURE - SAY SO! There's no harm in saying, "the name you're giving me doesn't ring a bell. I remember THIS email address, phone number, User ID name, or whatever in connection with this name, and NOT the one you're giving me."
It's OK to be in doubt, and to SAY SO! It's better to say you're not comfortable in vouching for the man than to do a 'yeah, yeah - sure, sure' and then find out you're responsible for setting someone up and causing lots of unnecessary, serious harm to someone.
