My buddy who went to one said he was glad he could speak/understand Spanish. It made his experience a better one than the guys who were English only.

It's helpful if you have a basic level of fluency in Spanish. Otherwise broken English and hand signals come into play. Did I ask for a BJ or call a safety blitz? A guy I know who went to a place like this recommended waiting until you get there to select your companions.
They all work a little differently so I would recommend you look for boards that discuss these kind of resorts.
The original poster was pretty vague about what he's looking for in an all-inclusive (going with his wife? A provider? Looking to pick up local women?). So unless he clarifies, no one can really make a good suggestion for him.
But as far as language goes, the Caribbean is a super diverse region. I've been to islands that are primarily spanish speaking, as well as islands that speak French and Dutch. Generally, islands whose economy centers around tourism will have resorts which English speakers can communicate fairly easily

and as Sticky already stated, most of them are in the DR, with one in Margarita Island off the Venezuela coast, and a few other places, Costa Rica also having a couple of such places.
I have heard mixed reviews on most of these places and in my experience, you can get a MUCH better deal "ala carte" as the local talent can be had for much less than Americans are accustomed to. Typically what you get for a around a grand a day in an all inclusive place is a Hotel room worth a little over a hundred bucks a day, food and drinks worth about another hundred bucks or so, and 24 hour companionship worth about two hundred bucks a day, round it all up to a value of lets say $500 per day and you are paying twice the price for the convenience factor.
As you might have already guessed, I prefer to "go native" lol

Viceroy in Anguilla. Straight out of a James Bond flick. Been there 4x including grand opening back in 2009. Never less than incredible. But as far as ass, it's strictly roll your own.

and if money is any object at all, you can do much better by booking your room, meals, drinks, hookers ala carte. Usually at least half the price, in some cases a third of what these "all inclusive" places charge.
One other thing to keep in mind, you don't run the risk of having to compete for the handful of hotties that get scooped up first, leaving you to have to settle for the 5's and 6's that are left over. In most of those places the guys that book the priciest packages get first pick of the girls, sometimes what's left is hardly worth the flight to another country as you can find similar talent for a $100 on Backpage.
look up blackbeards adult resort it's near purto plata. i have read some good reviews never gone tho planning to when i have some time. go to their website and send them an email asking to send you pictures of the girls since they are very low key and don't have any on their site.

Tried one of these All-Inclusive Resort Ventures to D.R. a few years back.
Pros: Price very reasonable at time; Over 20 providers to choose from prior to booking
reservations (allowed to choose 3 - may or may not get first choice);
Provider stays with you 24hr/day for total number of days of vacation; allowed to
switch provider with 24 hour notice at no extra charge; no hidden fees
Cons: Providers may not speak English (will know when choices are made); method of payment;
Contact person at resort area not same who talked with when making reservation; Personal
representative to would meet at airport did not show (sent someone else); only contact to
resolve problems was via phone contact
