and there is a lot of research out there to suggest that if gents did pelvic floor exercise they would in many if not most cases not need the blue pill

I'm new to the hobby and in my late 40s.  Sex has always been a curiosity for me but I haven't made it a priority until recently.  The problem is I've never been able to pop except during the occasional wet dream which is rare...only once or twice a year, if that.   I can get erections (with precum) by viewing certain porn (and while being with a provider) but achieving actual orgasm while awake has been virtually impossible no matter what type of stimulation I try.  I've found that sex with providers can be very pleasurable for me even if I don't pop, and I'm OK with this if I have to live with it, but I'm wondering if there are treatments (or medications) that could drastically improve things?

first get thoroughly checked out to make sure that there is no possible physical cause -  but don't be surprised if your journey does not eventually take you to a therapist of some sort....

I suppose it is possible that for some reason your pelvic floor muscles are so weak that you can't actually orgasm but I doubt it....

I hope you are able to enjoy at any rate.

I once went to a urologist for a particular problem and a female physician's assistant gave me a prostate massage to relieve pressure. Oh my! She was youngish and a buxom redhead. To this day, one of my masturbatory scenes involves undressing her and fucking her on the examination table. I gloriously climax every time.

BTW, I've been told by the same PA that it's not good to keep it bottled up. Climaxing is necessary for your health.

I agree with Markus Ketterman.

You should also explore alternative/ holistic treatments and medicine.  Have you looked into tantra yet? Tantra builds your pelvic muscles and gets everything moving as it should. My sessions allow for a deep state of relaxation so you can be in the moment, which is tough to find these days. If I don't peak your interest there are plenty other skilled tantrikas on the board. I hope you find relief and feel free to private message me if you wish.

especially in a bound angle position,  in proper breathing and breath control,  hydration and nutrition, when it comes to sexual wellbeing....

This is definitely a question for your doctor.  It could be something as simple as low testosterone which is easily correctible.   But no way for anyone on an anonymous review board to give you an accurate answer.

Nick's typo still stuck with me when he meant to say "boink her" but auto-corrected to "boil her".
Boiling a girl certainly does sound fun though, but I'm guessing we'd have to have a stove, propane, and some boiling water. lol

Seriously though, go to a ejac specialist doctor who knows what they are talking about. Not all doctors know what they are talking about. I'd only see a doctor who specializes in this.

I have had some similar problems but able to ejaculate after doing some life style changes. This is what helped me.

1. get a full physical heallth check up.  make sure your hormones and other things are within normal range, if not get yourself fixed up asap.  you may have an underlayig health problem causing this.

2. i have switched to SKYN condoms or try Female condoms.

3. I run, job, hike, trail run... do intense cardio

4. Sleep well for several days before the session

5. Hydrate well the whole week

6. use lots of lube.

7. try different providers of different types, it's really odd and defies common sense but it's sometimes the least expected the provider who can get you off easily.

it just seems odd to me that no type of stimulation has EVER put me over the edge, even when I was younger.  I've definitely had some amazing wet dreams which leads me to believe I'm physically capable, but, as I said, they're all too rare.  The yoga and tantric message suggestions sounds very interesting and.I'll definitely consider that, as I've definitely been able to be "in the moment" while dreaming. I may get checked out by a urologist at some point but I'm still hopeful I'll have better luck over time as more provider opportunities present themselves.   Thanks for the insights!

A visit to a good tantrika might be just the ticket to getting past this blockage.

Also follow the advice on rest, hydration, and nutrition to which I'll add checking on your Vitamin D levels.

My doctor found mine low a few years ago and so I started taking 1,000 IU/day and I think that has had a great positive effect on my sexual health.

The stuff is cheap and what could be easier?
