I'm very experienced with urethral sounds, I strongly advise anyone playing with sounds be very knowledgable about proper sterilization techniques and sepsis. Ladies specializing in any invasive procedure should be able to fully explain all techniques to you properly as well as what sterile procedures they are utilizing to keep you and them safe. Certain scenes do require a makeshift sterile field which you should always ask about.

When playing with sounds and/or catheters, I strongly recommend that "safewords" be very simple; no "red", no "yellow", nothing like that.

First of all, the one who is administering should be checking in with the "victim" very frequently.  As for safewords, there should be only one: "Stop".  This game is really intense, can be great fun, but also involves more than a typical amount of risk.  Being cute with safewords is too much to expect of anyone in a situation like this.
