I was in a BDSM relationship at one point with a Dominant who I trusted and enjoyed playing with very much. However, he still managed to abuse this little pain slut.

He didn't do anything which was obvious as abuse, we both enjoyed it very much. However, at the time, I was new to the kink scene, and as such, I was still learning my physical limits. Unfortunately, I can literally bliss out while someone beats me to a pulp... He decided that, instead of helping me to recognize my limits, he would simply push me further and further each time we met to satisfy himself.  

Yeah, I ended up getting injuries which never should have been allowed to happen in safe, sane play. But I learned a lot from that experience and haven't allowed myself to be abused like that since.

Ultimately I told him to fuck off and haven't seen him since. xD
