Nothing is better than the "real thing", but when it is a toy, make sure it is as life like as possible ... stay away from hard, "unforgiving" objects if you are planing on being "pounded". There are some amazing silicone and other flesh-like materials out there, with some specifically being designed for anal / prostate pleasure. Also make sure the Domina is using a condom, as so many toys are a breeding ground for germs if not properly cleaned / sterilized. It doesn't get pushed aside, but if the provider is pounding down and forward (ie the dildo is pointed at your lower sternum) then the tip would press through the rectal wall against the prostate.  If it goes too hard then it can do some damage and you will get some bleeding (most likely when ejaculating).  The bigger risk is damage to the rectal wall - tearing or perforation.

Every guy is built differently inside, so it's an angle thing. How deeply she's penetrating, at what degree and what position the guy is in affects this, as well. In most cases, the prostate is receiving direct stimulation.  The guy's comfort level is what should dictate the action.  It's not supposed to hurt.  If it does, you're doing it wrong (not enough lube) or there may be a medical issue that needs to be addressed. I find that most direct and intense prostate stimulation comes from either digital manipulation or the njoy pure wand. Strap-on definitely stimulates the prostate as well, but it's often not as direct.
