Since you don't like it long and hard (when it comes to questionnaires), here's a document that seems to be about the length and depth you were searching for.  See what you think of this.

Honestly, I'd start by asking if the other party is okay with marks, and, if so, for how long.  The answer to that, if it's "No marks", is going to eliminate a TON of things right off the bat.  The aforementioned 50 page document might be down to 2 pages after that.

I know i've seen BDSM questionnaire forms that allow you to communicate to the person your having a session with what you like/dislike have tried/might want to try, really want them to try but don't want to ask and so on. I was looking for one online but everything I've tried Googling comes back with a really short half page form or a 50 page document with with a required essay. Does anyone know of a form that's in the 1-3 page range?

It's a bit on the short side because I also like discussing my partner's interests via email or vis-a-vis, and of course it's geared towards a P4P session, but I think it covers much of what I need to know about my partner to please him or have him please us. It builds a nice foundation for getting to know each other. I like the well thought out and genuinely interested in BDSM answers the best, of course!
