The hook isn't a stretching device.  Hell, there are plenty of much bigger devices for that.  Compared to dildo's and plugs, it's actually quite small.  
   
 When one was used on me, it was attached by some sort of twine to a collar around my neck.  The wire was taught.  This kept it from slipping out of my ass, and also, make the wire feel like it was an extension of my nervous system.  Every time the lady touched the wire, I could feel it all the way through my spine, and my prostate.  It was one of the best experiences of my life.  
   
 It probably helped that I was blindfolded and had no idea what she was using on me until after it was over.  :-)

One of my favorites is to put (or be put) into a bondage dilemma.  The ball end is slowly inserted into the rectum.  The hoop is tied to a rope which can be tied to the subs hair so the he/she must keep his/her head back because of the sub relaxes the neck muscles, the ball pulls on the rectum the prostate or the internal vaginal wall. One of the best is to have (or be) a restrained sub with say nipple clips tied to a cord.  The hook rope is looped on a hook in the ceiling over head and tied to the cord from the nipple clips.  The hook rope is pulled tight enough so the sub is on his.her toes.  If the sub relaxes his/her calf muscles, the clips hurt more; if the sub gets up on the toes higher, the hook puts more pressure on the prostate or rectum and internal vaginal wall. To add to the stimulation, one can add a spanking or whipping.
Before this is done, the dom(domme) and sub should know each other well so that it goes just far enough to be wildly stimulating but not harmful or negative. A really experienced dom(domme) can do this to wonderful effect.

I have a great deal of fun with mine, personally I enjoy using one with a stainless steel ball on the end. I usually tie it to my subbies collar and then as mentioned above they have they control over it with their neck muscles while I have control over their nipples and other naughty bits whilst they are otherwise restrained. Anal hooks can provide a lot of pleasure if used properly in restraining.  I built my own private dungeon and I installed anal hooks as an integral part of the rigging system.  Every sub woman I have used them on has enjoyed them immensely, even those who expressed initial trepidation.  But like everything else in our corner of this hobby, you need to be cautious.  But also, think about all the times you did something for the first time nervously, and then grew to love.  Open your minds (and cheeks) and enjoy.
