I like advance booking but I require verifiable information.  It would help if you have reference and employment information we can verify.

Totally new to this as I don't live in the USA.

Have sent a few e-mails trying to make an early booking for an appointment in LA in a couple of weeks but have been getting no replies - is it inappropriate to try and make the booking too far in advance? Or is  there some information I need to provide in the e-mail to indicate above-boardness?

that you appear difficult to screen. Non-USA residency will prove more problematic for many providers. If you work for a big multi-national firm and your employment is possible to verify, I would think you can get through many providers' process.... but if not, you might find it tough.

Tabu is correct, some ladies would find it difficult to screen you, but rather than say so they just ignore you, not very polite but just the way some are. Most established providers prefer booking in advance, but many do not.

Also, different ladies have different preferred methods of initial contact which will usually be on their ad or website. Many I know will require you to fill out their contact page form first, or make a phone call first. Would be nice if there was a set standard way, you just have to search for what each lady prefers.

Thanks a lot for the advice everyone.

I'm anally-retentive and like to plan everything ahead but maybe this time I'll have to take my chances and just phone once I get to LA.
