Start the exchange now (email and follow-up phone call). Give her your info so she can verify you. Then, when you are ready to set a time and a place, your are in like Flint.

She can take her time verifying you, and let you know if she will be available around the time you are thinking.

If she does not normally provide e-mail contact info, then calling is probably her preferred method of contact. If you e-mail the info to her first, then when you call her, she will know to look for the e-mail, rather than trying to take down all the info on the call.

A well reviewed lady I am interested in has only a phone number on her main web page of her TER profile but has a reply link on her second Backpage ad.  I am planning to see her sometime in the next few weeks.   Should I initiate an e-mail exchange (maybe 2 weeks in advance) or just call her the week before I would like to see her?  Is more than one week in advance to contact too far away?

If she doesn't return the call, Email her also. Or you can email 1st, then phone 2 days later. I'd go ahead, and make the contact now.  Reconfirm the day before the date.

Two weeks is a pretty good time frame. If you wait until the same week, she may not have the time slot that you're looking for. As mentioned, you may want to start with an email, then follow up with a phone call.

I have an upcoming first date with a special lady...  I made e-mail contact 2 weeks in advance...  seemed like a lot of notice but her calandar was more full than I expected.  
The only down side is the suspense is killing me.

I am seeing a provider tonight that I contacted via email two weeks ago. In the initial email, I told her I wasn't sure of the exact day but gave her reference info so we could get that out of the way. She emailed me back two days later saying that we are set and to get back to her when I was free to see her.
A few days later, I gave her the date and she gave me the times available. Said to email her a few days before the date and she would give me her address and phone number, which she did.
