At any rate, if you give the money directly to your roommate and don't make an agreement with the doctor then he will have nothing to enforce against you. (But be careful about any agreements which authorize her to make charges on your card.) If the doctor is willing to extend credit to the young lady and she, shall we say, decides to just blow the doc off, he can't turn you into an involuntary co-signer.
As for her, there is nothing you can do legally to enforce your rights under this agreement. A contractual agreement to exchange sex for money is a contract for prostitution, and no court will enforce any contract that requires a party to commit a crime.
I suppose you could draw up a loan agreement and then agree to forgive the loan over time, but then she would be the one taking the risk because there is no guarantee you would forgive any of it. My guess is she won't go along with that one.
As for charging back payments to the doctor, that depends on how you make the payment in the first place. It sounds like your lady friend wants you to give the money to her, not to pay it to the doctor. After all, doctors don't usually have a procedure for accepting good faith payments. If you give her the money from a cash advance or a check drawn against the credit card then you will have no rights against the doctor.
And keep in mind that unless she needs a reputation for delivering sex as promised (i.e., she plans to become a provider), she will have no incentive to sleep with you after the surgery. It's a question of whether she will keep her word after you keep yours.
It all comes down to a question of trust. From your question, I'd say that particular commodity is in short supply in your house.

If she refuses, say okay, no problemo. Then, I'd tell her to leave, right then and there, and find some other place to live. Maybe she could find some way to make her own money.

I've narrowed down the crux of your post to that one sentence. My advice to you is to read it and read it again and again until you really, truly understand it. Only then will you know what to do.
If I wanted a "free" boob job, I would definitely sleep with the guy first (and make it good!) to get him hooked.
If he refused to fork over any money, it's just one night of sex that I wasted....but if he takes the hook, I've got new boobs. It's a small risk on my part.
