it still depends on what her menu is.  Typically I get asked something like what do you want?  Sometimes you'll be offered options.  If there's a language barrier hand motions work.  I've been told a response of "Everything" works...  but again, her menu may have limits.

I've always taken backward teasing of your nuts while you're on your stomach as a good sign.

For a place I've never been to, I look for a sign in window for table showers, or inside the door if there is a posted price list at the counter. If no indications, I just ask if they have one. I've never left unsatisfied from any place offering table showers. It's a big ice breaker for sure. Especially if you get wood. In my area there are litterally hundreds of amps within 30 mins. Word gets out which ones offer FS or just a happy ending. Through review boards or whatever. There's usually just a few that are steller go too's while others are quite satisfying, and a few that are just huge money gougers for the tourists. But most of the money gougers do not offer table showers either.  

When I lived in San Francisco years ago, all you had to know was if it was open or not Fond memories of one place with a lineup and a stunning young K-girl. She turned down my marriage proposal.
