Would like to see addition to Item #4, Submitting Web Addresses:  In many cases, a provider does not have two websites, included in the instructions there should be guidelines indicating how to fill in that area.  While guidelines on form itself show a * for this field - not required, in past three reviews I have submitted I got a message must be filled in... I did use http:/// to get around the field, but I know many folks have had issues with this from discussions on the Reviewers only board.
