Seriously, my feeling on dental dams is the same as CBJ, if that is what's on the menu I would rather do without.  And no, I doubt many ladies would be offended if you approach it right and don't make her feel like it's just her.  If you decide to go forth with this abomination (grin) you will likely have to bring your own, as I don't think many ladies "stock" them.

I mean, yeah okay, if you brought one I'd let you use it, but I just don't think it would do anything for me, so if it's pleasure you're aiming for you might be better off skipping that one. Would I be offended? Nah, not really. There are guys who don't want to DATY at all, whatever.

So far as herpes- always be sure to look closely at her lips to make sure she doesn't have any fever blisters (i.e., herpes), which can be transferred oral-to-genital and vice versa.

Use dental dams.
Put lube on her side, and if you want to, flavored lube on your side.
They also make garter-like garments that clip on the dental dams, so you don't have to hold it up the whole time with your hands.

I have used dental dams before.  For me, they feel the same when I receive oral, but when I've given oral to a lady with a dental dam, it's definitely a different sensation.

Maybe if you do it enough, you can fetishize the latex...
