Read that cover to cover. Also, be on your best behavior and be respectful. Think about how you would call to schedule an appointment with a doctor, only more discreet.
In terms of price per hour for service, expect to pay $250-$400 per hour for decent service. By that I mean the girl has booked a nice, clean hotel (if it's an incall), she's a nice, clean person, likes what she does and does it well, etc. I usually stay in the $300-$350 range.
Wondering where to look for providers? For a start, look on your regional message board here on TER and see which providers have posted their schedules. I know a number of very nice escort who charge in the $200-250 range.
