I guess this subject is 2nd only to price when thinking about the most exhausted topics on these boards but please stay with me.   Clearly hygiene might be the most frequently mentioned piece on most of the ladies and/or agencies sites.  I do not doubt that unfortunately providers are probably faced with some scenarios we couldn't fathom.  
That said, I have always taken the extra time to be as impeccably groomed as possible when arriving for an appointment.  Even if freshly showered, mouthwashed, etc.  but the lady asks for another round of more of the same, I am game.  I figure if it makes the lady more comfortable then we will both have a better time.  Here is where the BUT comes in.  I have made two appointments in the last 5 days - one indy - one agency - both highly reviewed.  Due diligence provided me with no information that would suggest either might be a disappointment.  Unfortunately both had less than ideal hygiene from breath, slight body odor and most damaging - not so attentive management of lady parts.  
With all the time spent trying to coach clients down the right path, how is it that this can still happen with highly reviewed ladies demanding $300-$400 an hour?

I find this with about one out of ten encounters. Something that makes you go ewwwwww. I just never go back again, and figure I'm lucky to have a pretty good batting average. If it makes you feel any better, with amateurs it's much worse.

and if we're being totally honest, it doesn't leave the client with much opportunity for recourse.  If you mention this type of thing in a review for it would likely be viewed by other providers as you being a douche and in the case of the agency, they might likely blackball you from seeing their other girls if you left a bad score

If you are honest in the review why would other providers black list you? Or do you find all providers to be poor in hygiene so all your reviews would be low scoring and mention the poor hygiene?  Providers are going to do what the hobbyiest does, look at the pattern of reviews and not just one review.

And as for someone mentioning amateurs are worst...
I believe its purely upbringing. The older I get the more I realized my bathing habits are not as common and at times even deemed weird. But then again my parents are from the tropics.

How do you raise the topic without making the situation really uncomfortable?  IDK

Not sure what agency protocol is but you could contact their booker and make a private complaint that hygiene was an issue (it may have been a one off thing for the girl, who knows) in lieu of posting a scathing review. This would probably save you from being blacklisted by the agency.

As far as indy's go I would just put it in the review. If someone left me a review and said that my hygiene was not up to par you bet your ass I'd be perfect upon my next appointment. Sometime's we miss things or don't think they're that bad but in reality clients think otherwise. Constructive criticism would definitely save her from making that mistake again.  

I would start a habit of offering a provider gum or a mint as well as eating one yourself when you get to the appointment. If you're concerned about BO in the beginning take a shower and ask if she'd join you. Hopefully you don't run into this again
