Most reputable ones are going to want to see your phone number,
want your real whole name and may want your employment info.

She will want this for her own safety, but I would urge you not to give
the info if you are not comfortably doing so, nor would I give it to any
that do not have a couple pages of reviews.

I have heard there are verification sites you can use too.  P111 I think is
the one everybody uses.  

Once you are around the block you will be a known quantity and will not have to surrender  
so much info.

....honestly a reputable provider more than likely wont  see you without filling out her reservation form. The personal info varies as some may only want your name and occupation, while others may want things like your address and copies of your id. Your discretion should be her top priority so do not be afraid :)

I have most likely seen more providers than anyone you will ever meet and I have NEVER filled out one of those insipid booking forms, nor will I ever do so.

If you have anything to lose if a provider goes BSC on you, be VERY careful how much ammunition you give her to use against you some day. That is the entire purpose of gathering as much information about you as possible, SHE feels safer because she knows she can destroy you if you ever get out of line.

A previous poster hear said check the provider's information form on her site to their credit they also said don't give out any information you aren't comfortable giving out. I won't fill out the provider's web contact form. It's on the web. You have no control of it once you submit it. I will give my info by e-mail. If the provider isn't ok with that I move on to another provider.

Using a verification service such as P411 (not P111 as stated in a previous answer which I'm sure was just an unfortunate typographical error.) is a good way to deal with giving out personal information. You give it to the verification service once then you don't give it out to every provider you contact. You just give her your P411 so she can check there. She doesn't see your information.  

Note some providers that use P411 still ask for additional information. You decide if you're willing to give it.

A provider can ask for whatever information makes her comfortable. It's her right to do so. I won't argue or complain if it's more than I'm willing to give. I just move on to another provider.

I don't like the idea of the "blind submission form"  either and have long since discontinued the use of it.  I prefer an email wor direct communication by phone so that the prospective client may be comfortable with submitting their info.  

411 and DCK are both good platforms or clearing houses for Provider and Hobbyist but I still require a little info as do many others, sadly it seems that ANYTHING can and WILL BE breached by those with the wrong intentions.  

Honestly, I believe when you talk directly to and find the Provider  you are most comfortable with you will probably provide whatever info she requests.  As for the earlier post about the Providers with the high review counts being the SAFEST to give your info to, keep in mind that not all encourage reviews.
