Simple question, do providers prefer guys in nice button down shirt and dress pants or jeans and polo? It's given that both have perfect hygiene. Just curious which style is most appealing.

... If a man shows up looking nice with a button-down shirt and the cufflinks, I somehow feel more respected? Ladies, do you feel the same? It's just nice to have someone put in a fraction of the effort that we all do.

That's how I feel. I make an effort to look good and very presentable. Clean shaven and light cologne and business casual outfit and of course newly shined shoes. Just was curious if providers noticed that some of ur make an effort as well.
I will always dress with nice jeans, slim fitted and dark wash, not some baggy looking crap, long sleeve button dress shirt and very nice shoes, my favorites are Allen Edmonds I have several pairs and sometimes a nice watch
And the shoes DO make a difference. You are sexy as hell
