You might want to look at a recent thread on the national board about the wisdom/need for a provider to incorporate.

The only reasons to incorporate are if the provider has a business other than "providing" which she wishes to incorporate and wishes to protect her personal assets from liability for the corporation's debts or to avail herself of personnel benefits though most of those are available to a sole proprietorship. (Ex. medical, 401k)
An LLC is usually not the proper vehicle for a small, personal service business.
