1 - For incall at a hotel then you do not pay for the room unless it's been clearly agreed in advance that you will pay. If you request an incall and the escort says it's in a hotel then perhaps she's touring or doesn't work from home and it will be at her expenses.

First call is to set up appointment; at that time, you are told the general location.
You get yourself to the general area at the time of the appointment, and make the 2nd call which will get you the specific details of the location (exact address / appartment number, room number, parking lot number - lol).
As for incalls, you don't pay for room charges in hotels.

I use the two call this way:
1. Call me as you are leaving ( for some this may be an hour drive) and I'll give you the thruway exit or general locale and general directions.
2. I guide the guy down the street to my place.
If it's incall, the provider will usually check into the room before you get there so that she can prepare.
