Most ladies will view that as a reasonable request. You seem to have found the key...ask nicely.
Absolutely not too much at all! I enjoy that as well and women I've asked have been happy to oblige. If you happen to ruin the stockings or break a garter, my feeling is that you should add enough to the donation for replacement. I also like to take the woman shopping for lingerie before-hand then have her model! Simply awesome!
I know the sex should be exciting enough...but the added element of heels, and bra and panties just makes it all that more intense!!
I often have clients ask me to leave items on. The only time I will remove the heels is when things get heated up. High heels in a boudoir can get dangerous... and its nearly impossible to stand on a bed in them... plus the damage they do to the sheets.. lol
Guys are very visual and most providers want to provide the most visual stimulation (among other stimulations) to their clients.

During a change of postions post-69, I caught a spike heel right in the corner of my eye. Did not draw blood but I did get a distinct welt.

You are so right about being visual. If 1 is stepping out of a freezing swimming pool and 10 is a paralyzing O, I can get easily reach an 8 just by the sight of a gorgeous woman dressed to kill...

Try not to be creepy, and give as much notice as possible, especially if there's something specific you've got in mind. You should also be understanding about any possible consequences; for example, I once asked a lady to leave on her stockings (and she was happy to comply), but after we'd finished the second round, we were both had worked up a sweat, so she wanted to take them off, which was OK with me.
If there's a specific outfit you'd like her to wear, remember that many ladies keep their places at a temperature comfortable for unclothed lounging about. She may not be crazy about letting you see her sweat through something (or she may worry about odor issues). It's just a natural phenomenon, but so is farting, and she probably wouldn't want to let a big one rip in front of you, either.
Reasonable requests expressed politely will rarely be denied. If you spring something on her at the last minute, she may be unable to accommodate you, though -- she may have left the "fuck-me pumps" at home, or just not have a certain sort of clothing in her closet. Give her enough notice, and be sure to let her know if it's important enough that you're willing to provide it yourself. I've seen a few guys whine in reviews about things like "I asked her to wear fishnet stockings, but she forgot, and I didn't even want to go through with the appointment if she wasn't going to have them on." She can't read your mind, and if a given thing is a session-killer for you if it doesn't happen, she needs to know.
