Remember, the ACA to a large extent depends on people who can afford health care paying for it. Therefore, it is geared to people's ability to pay.

Think of Medicare as an example, but not everybody has to participate. Is that what you want? Do you want Universal healthcare coverage where everybody gets HC coverage? Do you want the UK version, German version. How about the VA for all? That's a govt provided H/C delivery system. There are a myriad of variations. That's one reason I can't be onboard, because I can never get a specific answer on what you guys really want. Plus if it's Medicare for all, then you guys can never answer how tp deal with fraud, abuse and waste. Today it's $80B in annual waste.

First, "Single-payer" is a myth. France, Canada, Britian, Italy, Sweden, don't have it. They have a system where the state provides a minimum that the state defines as sufficient. However, there is also insurance and private doctors for people who can afford it.
There is a term for it over there. It is called "Going private." This is not a term I made up, but part of their medical lingo.
THe main problem with a true "single payer," is it is also a "single decider." The entity that pays the bills decides what you can get. THis means that if there is a new treatment that is "experimental," it may be a better medicine, but it is expensive the first years while tens of millions of dollars in research are recouped.
However, on a cost basis it is therefore not worth it, so the state won't pay. The problem then becomes fewer new medicines are developed because the cost is too high.
Obama himself spoke about "smarter spending." That necessarily means someone has to decide what is smarter, and if you have single-payer, in a true system, it is a committee in D.C. or Ottawa or London.
If you want something the committee hasn't approved, tough luck under single payer because no one else can pay for it.
Again, that is why the U.K., Canada, and every other "Single-payer," have private insurance for those that can afford it

It's not tax fraud. I specifically know a few early retirees, i.e. not yet old enough for Medicare. They have to buy individual plans. I think they are in there late 50s. The lowest cost bronze type plan, non subsidy, runs about $1050 a month in Southern California, specifically in a zip code that is considered affluent. Zip codes are important in the calculation of a subsidy. This couple has a home, and a 7 figure investment portfolio, think IRA and stuff.
The magic AGI (1040 form) number is $62,500. If you are below that number for a couple, you get a subsidy. Now the subsidy is based on income, age, and zip code. Even though this couple elected to go with a high deductible bronze plan, they were able to adjust their income legally so it would be just below the magic number of $62,500. They have a pension, but don't take capital gains, i.e. defer income. They just made sure to stay under the $62,500 threshold.
Even right below the threshold of $62,500, the subsidy comes out to $950 a month. That means this couple, and you could consider them wealthy from an investment portfolio perspective, is only paying $100 for a bronze plan. The taxpayers are footing the bill to the tune $11,400 a year.
Just want to make sure you are OK with that. Everything has unintended consequences.

Yes, it would be tax evasion not to declare income, but since we probably agree that many will not, then they will also be getting subsidized medical care that they could otherwise afford and don't deserve, and you will be paying for it twice.
