It really does have to be very tight for it to do it's job. Sometimes putting it on when your soft doesn't work as well because you can't get as much blood flow into the penis. Using a penis pump that stretches out the cock ring and allows you to slide the cock ring down on the penis makes it easier to get you at your hardest.
