Sorry your are leaving. I'll miss your advice. Though this is the first time I've written for my own problem.
About two months ago I began seeing a new lady who is very sexually liberated and aggressive. Mirrors my desires perfectly. I'm 66 and she is 50. Although I need Viagra or Cialis to assist with an erection. I've not had any problem getting hard with her most every other night or so, and again in the morning. But in what must now be fifty lovemaking sessions, I have only come twice: once vaginally in doggy, and once with her masterbating me.  I am able to give her many orgasms in any given session, but she is frustrated that I can't come inside of her. I think she is viewing that as some sort of failure on her part.
From self analysis, I think I'm not able to hold my concentrated focus long enough to gain the intensity to come. I work out a couple times a week and feel in reasonable condition, but when I try to maintain real intensity long enough to come, I eventually lose it.

You mention that you do use Cialis and Viagra, and I'm afraid there's the rub.

These products, while assisting in the erection department, do have the side effect of making orgasm and ejaculation more difficult to obtain.

It's important that you share that information with your partner so that she does not feel that she is somehow at fault.

You might discuss this with your doctor and see if a lower dosage might not allow you to have orgasm more easily without loosing the erection.

Otherwise, take some solice in having half a loaf instead of none.

You mentioned that you work out and are reasonably healthy. I knew someone once who had almost exactly the same experience you wrote about. He went for his annual physical and found out he had a blockage in his carotid artery. He ended up having a procedure to fix the issue and has not had the issue since. Im not saying this is what is happening with you only that it sounds exactly like what my friend went through a few years ago.
