The most obvious difference is that a hermaphrodite has both male and female genetalia below the waist. Generally, but not always, the testicles if present are still internal. Many times they have some combination of ovaries and testes, usually not exceeding a total of two.
In my experience a hermaphrodite has both sex organs. This can sometimes create problems. The lady I had experience with was like this, and could not get hard, or be penetrated from the front.
