About safety but don't be offended if she doesn't want to. For me it is a NO. A hard no. I do not like latex taste or fake flavored covers. It's not enjoyable for me and would drastically lower any review I may get lol. But I always provide the covers for other play. Not an issue there. Nothing wrong with asking for covered. I do a wonderful relaxation massage when in town if you wanna try something new.

Trojan of any sort... although the bare skin one seems to be a favorite as it is thinner than most.

what would be the motive for a provider forgetting condoms? are you saying they try to avoid having sex, or are you saying they want bareback?

Sometimes they do forget or maybe they didn't have a sufficient quantity. If a guy has his own, it would be wise that he not open the box.
The time you should be concerned about is when she has little or no reviews and you're TOFTT. Perfect scenario for a Grab-N-Dash if you're not careful.

Having your own condoms is a really good idea if some providers are not bringing them.

Like others have said, I would bring my own supplies. You may also want to seek out non-GFE providers if you only want covered BJs. The majority of the non-GFE providers will only do covered blow jobs. Most providers will have this in their profiles or you can read this in their reviews.

I personally would not be offended at all. If there is provider who would get offended by this, she should quit the business....And to be on the safe side, I planned on being the one who provides protection.
Other clients want covered BJ and it's not a problem at all. It's true some condoms have a terrible taste. Some of them make my throat numb and I think it's spermicide, but I know which ones are fine and those are the ones I use for Cbj.
