I have read clinical research indicating that protected anal intercourse (sex with condom) is a higher risk activity than unprotected oral sex -- without ejaculation. Unprotected oral sex with ejaculation may be relatively equivalent to protected anal sex in clinical percentages of hiv transmission. This tells me a couple things. Either a lot of condoms are breaking or people still haven't figured out how to use them properly (squeeze out the air, leave room at the tip, roll up completely, don't mix with oil lube, etc etc). A simple "use a condom" is not a cure-all or a prevent-all here. And my own non-clinical opinion is that dirty or shared needles - be it for drug use or silicone injections or any other purpose - are still are far greater cause of hiv spread than any kind of sexual activity.
