Just leave. The hotel will automatically check you out and send you an email with any room charges the next morning. There shouldn't be any reason to deal with the front desk.

I will be seeing a provider that only does outcall (and only to specific hotels). It is my first time doing this; up until this point I always say providers who offered incall.  

I intend to check in at 3:00 see the girl and check-out by 4:30ish. I am worried about awkward questions when I check out. My plan is to put on my sad face and claim that my plans fell through and that is why I am checking out a hour or so after I checked in.

Do I need to be on the lookout for anything or do hotels have this happen all the time? Is my excuse a good one or should I go another route?

Good advice. leave key card..Don't leave no evidence in the room condom and take out trash....

at 3pm, what are you going to tell the provider?

I usually check-in as early as I can then read or do work before taking a shower prior to the provider arriving, usually around 5 or 6 pm. Why rush things when you can let the anticipation build?

used towels in the bathroom.  A small tip for houekeeping with the key card.  Good idea leave no personal trash like condom wrappers in the room (though you can drop a paper bag in a public trash can elsewhere.)  I would leave typical traveler trash in the room...  soda cans or cups, wrappers from food...  
No one will know what time you departed... no need to go near the desk.  If you are OK with charges on your CC, no reason to check out.  Some guys do check out & pay cash but you still need a CC in case there are other charges or damages.  I'd think that would be a red flag.  Having sex in hotels is not illegal & there are lots of trysts...  it's only illegal if she's paid.

To the OP, when you leave the room, simply leave. The less mess you leave for housekeeping the happier they are going to be.  The one thing HV said is "having sex in hotels is NOT illegal" leaving evidence of sex isn't going to raise a single eyebrow on the part of housekeeping.

Even leaving the key card is unnecessary, they cost way less than a penny apiece and housekeeping is simply going to throw it out with trash anyhow.  

Now the rules for how a hooker should leave her room are a lot different, hookers really should not leave evidence of multiple sex partners, those types of things could "possibly" come back to bite her some day, but for us guys, don't give the way you check out a second thought. Virtually no one who isn't settling up with room charge bills or the like ever bothers checking out at the front desk anymore.  Things are automated for a reason, less hassle for you, and less work to do for the hotel staff. Don't worry about a thing.

It may be overkill but it's how I leave a room whether I had a guest or not.  I always leave the bed open so they know it was 'slept in' if indeed we were between the sheets (often not the case).  I always wonder if house keeping really changed the sheets.  
I think it would be more suspicious to have nothing touched in a room that was rented.  I want the room to look like it had normal useage...  and the things mentioned were things I typically do in the room.  Sleep, shower, have a late night snack.  
Oh, yes, open a bar of soap.

I have rented a lot of rooms where I have never even set foot in them, much less slept in them.

Ok, maybe not a LOT of unused rooms, but more than a few times I have rented a hotel room, thrown my luggage in them and then "got lucky" and only returned to pick up my bags. Not to mention the many times in gambling towns like Las Vegas where I have booked a room at one hotel only to get drunk at another and never making it back to the room I originally booked.

Once again, let me ask you, why do you care about looking "suspicious" long after you have left the property? No one cares if you didn't sleep there. There are a thousand "non hooker" reasons for not sleeping in a room that you booked for the night.
