It wasn't always that way, but they changed the policy a couple of years ago. I kind of like it that way. I don't really want hundreds of okays. I usually request about one new one a month, just to keep a handful of "current" okays to make screening a breeze.

A TER white list can be requested, or a lady can simply give you one of her own accord. I think most women would agree that a P 411 okay is worth a lot more than a TER White list, there are a LOT of fake White lists, until recently I had several White Lists from women I had never seen. This kind of thing is eliminated on P 411 by the rule that say you have to "request" an okay.

I am a very newbie. Curious about this white list thing. I just got an OK on P411 from my first provider. Is that as good as a TER white list entry? Also, on P411 I had to request it. Here on TER is it just something a provider does without any kind of request?

Will vary from lady to lady. I'm sure a few of them will chime in here. FWIW, I've seen a couple on here say the whitelists are meaningless to them. But that's not to say they always are. Some providers will accept them. Not a bad thing to have in your pocket for sure.  

There may be a few ladies who would just do a white list, but I think for the most part it's something to ask her for. The most common advice is to drop a brief note one or two days after your date to say thank you and ask for a whitelist if she didn't already.

Never been part of 411, but having been given 4 whitelists from ladies (without ever asking), seemed to me to be a nice compliment.  I will also tell you that they have always been exceptional dates and not only from ladies I rated high numerically.  So, I can't imagine that a whitelist is meaningless or doesn't have value in the screening process.
