To be FBSM then some sort of a HE (happy ending) is expected...  that would be a Hand Job.  A strict massage (typically by a LMT) does not offer that.  
 
I have had Favs that were HE only but all allowed some touching...  pretty much necessary or you're going to be there a long time trying to make him happy.  About all went topless, most full  nude.  Every one allowed breasts &/or body to be touched.  Intimate touching I'd say most but not all.  This is 'strict' FBSM.

Some will step up to a BJ from a HJ...  and can charge a bit more.  
Some will do FS for an additional fee.  Call it an extra if you will.  DATY is likely if a gal allows extras.
My longest Fav was a FBSM + extras gal (after she got to know you).  We pretty much did everything.  She had a set fee for the "massage" but expected a nice tip for the extras.  She would never set a fee for the extras (most do) which was very stressful as a client.  No one wants to over pay but if one does not 'tip' enough I felt I'd likely not be accepted back.  I guess I did ok as we saw each other over 6 years.

Some FBSM Gals ARE pretty GFE...  others not.  Some (with BFs I suspect) feel that kissing is too intimate.
