Doesn't anyone remember condom machines in almost every service station restroom?  I think it was 25 cents, though I could be wrong on that.
I guess the most ironic thing about this is I was visiting my son at a major university campus recently. I went into the men's room in the LIBRARY and lo and behold, there was a condom machine. Sorry, I did not check the price.  I guess the kids need protection when they take a study break.

Today I took the survey Fisher asked about.  One odd question was whether I had ever had a provider say it was OK to omit condoms for particular activities.  My experience (not a lot, less than two dozen over the past 14 years) has been that the unspoken expectation has been that condoms were required for penis in vagina or anal sex or for genital on genital rubbing, but not required for anything else unless specifically requested by either party.  Only one provider wanted to use a condom for fellatio, said so rather apologetically, and I didn't complain. (One other had the odd technique of sucking for three or four strokes and daubing the end of my cock with kleenex to dry up any pre-cum.  Whether she thought that was a safety method or was just to avoid the taste, I didn't ask.)  For all the others, bare fellatio and cunnilingus was engaged in without express verbal pre-approval.

reminds me of the very old joke about a young man buying his first condoms.  When the purchase was rung up, the clerk indicated it was $2.10 including tax.  The young man replied, "So that's how you keep them up!

I was with a well-known and well-reviewed provider one time who waited until the magic moment to stop and tell me that she had left her condoms in the car and it was okay with her if we didn't use one.  I instinctively pulled back and asked her to get them.  My recoil, especially given the moment, even surprised me.  I later went back and read her reviews more carefully and saw someone else had the same experience with her.
