Most ads say something like tips are always appreciated but never required. Many choose to tip agency girls (who split the main fee with the agency but keep all of their tips) more and more often than indy girls (who control the entire fee).  

For me, the percentage varies and does depend on service.  

I don't tip until the end. Sometimes I DECIDE after the first five minutes, but I still wait until the end :-)  As I am getting dressed for my final exit, usually near the door, I will peel off the tip and unceremoniously slide it into her hand or, sometimes, whatever clothing she has put back on (robe), get my final hug or kiss and disappear, back into the real world.  If I put it someplace other than her hand, I make sure she at least sees it so she does not inadvertently throw it away or lose it in the laundry.

mrfisher uses the "palm the tip, slide it down her arm and then squeeze it into her hand" technique. Sometimes I do that but I'm nervous about being sued for unlicensed use of the mrfisher method.  

An upsell is not a tip (and is a big turnoff). If, from her Profile and ads, you were expecting BBBJ and it starts out covered and you ask about BBBJ and she says 'That's an extra $amount.' that's not a tip, that's an added fee or surcharge. If you were expecting covered and you get BBBJCIM without even asking, that is tip-worthy!

 I also wonder when is the best time to leave it. Do I just include it in the original envelope or bring a spare? If I leave it in the original drop, what if she wasn't worthy of it? Then if I drop it after it almost seems impersonal. She might not mind, but I don't want to tarnish my squeaky clean imaginary image of her.

I usually don't do this the first time I've met with her. If I've seen her several times I'll consider it, depends on the situation.

If you elect to, keep it separate from what is required.

Keep this in mind, she'll appreciate you wanting to see her again if you both have an enjoyable time.
