depends on the situation.  Personally, I like to book my dates about a week in advance for a local provider.  If she's a touring gal, it might be more in advance since she might cancel if she doesn't get enough dates to make visiting my city worth her while.  Look at her website, if she has one, or read her ads.  See if she has any time requirements for booking.  Also depends on where you find her ads, IMO, BP girls tend to be a little high volume, so I'd book at least one or two days in advance, and ask for an early appt.

If she is a high quality provider, she will want sufficient time to screen you, so give her at least a few days.  Screening does not always go smoothly and quickly, esp if she has to wait to hear from your references.

How far ahead should you plan to make an appointment? I mean should I call a day ahead or more to be sure the provider will be available?

I would say maybe a few days prior. Depending on the girl, the day before or day of could be too late. Getting an appointment a few days in advance ensures a better chance of her being available and still leaving you both adequate prep time.

...three or more would be better. Once you've seen a lady then one day out could be possible. Same day can be very iffy but possible with a provider you've seen previously. I usually make contact 3 or more days out regardless.

If she doesn't get a good response to her plans, she might then just cancel the tour

Very busy high volume providers who don't have stringent screening methods tend to be first come first served so prebooking is seldom necessary and some times discouraged. For those who have a day job and those who require references or other verification, several days to a week or more is a good idea with a confirmation contact the day before you meet.
