I'd copy and paste your general and juicy details into a word document so you won't have to rewrite everything then close all tabs, windows, log out of TER and then log back in and try again filling out all the fields of her profile and copy/paste the details into the appropriate space.

I write the general and juicy details in a word processor program first. This way I can wait a bit, go back and see if I want to make any changes. Once I am satisfied with what I wrote, I open to browsers to her profile. Then, in one of them, I click on submit a review. When the blank review page is opened, I can copy directly from her profile into my review page. This allows me to make sure I have all the data that's required correct.
Remember, a provider has to have either a personal website you can post a link to, or an ad on another site that you can post a link to. If she already has a profile, this information along with an e-mail address should be there. If she has a phone number posted, she doesn't need an e-mail address. She just has to have some form of communication access available. If there is no phone number, an email address is needed.
