is a massage gal who agrees to "extras".  For which she should be compensated.  I say this as I broke into the hobby with such a gal (no qualifying ads so I never could review her).  She charged reasonably for the massage (she had been LMT at one time) but never would state a fee for extras.  I suppose she felt that limited her risk of a bust.  I tipped enough to brng the total up to the level I would have paid a provider for that service level.  
Providers are expected to be "all inclusive" except perhaps for trips to Geece.

Totally new to the hobby. Do you tip on top on the hourly rate? Give gifts? Nothing? Not sure what the expectations are going to be.

The girls are making a fortune on an hourly basis so no need to tip. Any professional gal will tell you they never expect any kind of gift or tip.

If you have a great time with a girl, the best "tip" would be to see them again.

though sure, a lady will appreciate your generosity.
I tipped on my first two dates, but haven't since (though I sometimes bring a small gift to set a nice tone for the date).
The common line given here is that repeat business is the best way to express your appreciation for the good time she showed you

Is your repeated business. Sometimes even bringing a bottle of wine or champagne is a nice touch.

I'm not sure how ladies working for agencies deal with being paid, but I'd assume that they are not paid what their advertised rates are.  Some of those rates are on the lower side as well.  I'd tip an agency lady, for sure.  

As for independent ladies, we set our own rates and we see every time of what you've paid, so I don't think tipping is necessary.  Don't get me wrong; it's always nice when it happens.  Small gifts are really thoughtful as well.  I'm always caught off-guard when clients do things like that. :)

Personally, I prefer repeat business and word-of-mouth advertising.  The best tip someone could ever give me is seeing me again, or referring other clients to me. :)

I find it unnecessary and awkward. However, I gave my ATF, an agency girl, a year-end bonus for all the amazing sessions she provided me throughout the year. It was hidden in a small gift so she didn't know about it until after I left. Point being, if you're going to do it be classy about it.
