it's their site, their rules.  Like a previous poster asked, why would you not want the provider's name on the review?  If she's
new, she doesn't have an ID#, so there has to be a way to identify her.  If she already has reviews, they have to make sure the review gets on the correct profile.

why is it that every time I submit a review the link requires the provider's name? and also why the contact information if you can just contact via the ad? I still don't get it

Why wouldn't you want the RA name and number right there instead of having to click the Ad link.

Granted, I always click the link as I want to see the pics but...

I guess if you include her ID number however that could settle it, but having the name helps to prevent errors such as keying in the wrong number.

In any case, as to your second concern, this is oft asked, and the response from TER, when there is one, is that by having to make reviewers go through and enter the same information over and over, they weed out casual reviewers, and get only those who are really committed to the review process.

I for one don't think it makes a whole lot of sense, but it's their sand box and their rules, and as life's little miseries go, this one is not so bad.
