1.  This would be a low level concern.

2.  If this is upsetting, then tell her you want to scrub and come back another time, for both your sakes.  Most likely though it is unrelated.

3.  Typical requests are real name, address, place of work and phone numbers for both.  If giving all this info is upsetting, then tell her that you'll supply some but not all.  She might or might not accept that.

4.  Yes, I would include that info.  Better to be safe than sorry.

I am feeling nervous about my first appointment with a well reviewed provider in my area.  I have done my homework the best that I can, and can't wait for my appointment time.  I have a couple of questions:

1.  I had asked for a quick phone call with the provider to verify that she is who she says she is, but she never did call, even though a text from her said she would.  What do I take from that?

2.  I know there is not guarantee's of an LE intervention, and there was a review of LE's in the area, at the time of one of the reviewer's appointment.  Just a coincidence, or should I be worried?

3.  What are the typical requests for information, if you do not have any references yet, as you are just starting?

4.  For any of you that have done any reviews of providers, would you include in your review if you had an LE encounter while leaving an incall?

I look forward to any advice, especially from any of you that have been in the hobby for awhile.

And you could read her reviews completely, because you have VIP, and possibly even PMed some of her reviewers you should be fine. If you see LE they are not there for you, it's the ones you don't see you that need to be concerned about. Unless you act guilty when you see a cop out on routine patrol he's not going to give you a second look.
