If you are seeking outcall, then yes, you pay for the hotel. I would recommend staying away from motels (i.e. doors that open into the parking lot) and low-cost hotels, as many ladies will shy away from those as they are associated with LE problems. Once you book the room, you would give the lady the hotel name and address and the room number, so when she arrives, she can come straight up to your room. It is also helpful if you give her directions to your room so she can come straight to you without looking out of place (i.e. "once you come in front door, the elevators are to the right of the front desk").

Seeking outcall seems to be much safer than going to her incall.  

I'm thinking you set up the date, book the hotel and she comes to you. You can monitor from the parking lot and meet her at the door. This may be considered extreme paranoia, but it's a thought, right?

Anyhow, if one were to go this method, you can pay for whatever hotel is in your budget. Just a shame it may only be used for an hour or two.

Yes, if you ask for outcall to your hotel you rent the room.  If you are staying at the hotel, there's no wasted money.  But if the only reason you rent the room is to provide a meeting place, then yes, it does seem like a waste when the date is over and you leave the room.  Do not make the mistake of giving the escort a key and letting her have your room.  If the room gets trashed it's you who will be billed for the damage.  On the other hand, if you get to know a girl well enough, you can ask her to meet you at a less expensive motel and save a few bucks.

Yes, this is the way to go.

Get hotel via priceline or hotwire.  
You can check in as far advance of appointment as you want (remember though most check in is after 11am or later some hotels).  If hotel uses room keys for elevator you will need to let the lady know and meet her somewhere to navigate the elevator.

If a large hotel/convention center generally one can take the stairs to second floor where the meeting rooms generally are located - this is away from the immediate front desk and you may feel more comfortable meeting the lady there and then grabbing elevator.
