I did some research on this and found several articles on this exact subject from very reputable sites. What I found to be the case was that the mouth is fairly safe place unless you have open sores in your mouth or some sort of gum disease.  

Herpes - can be a concern from unprotected oral, but only is a concern if there is a current outbreak. Usually, an outbreak is very appearent. Before doing DATY look around the area, and make sure it is really some where you want to put your mouth.  

Another concern that many people don't think about is staff infections, because of the trend to shave everything these days, if a lady shaves and gets her hair folicles infected (it will look like razor bumps), that can be transfered to you even with a condom on.

I am not trying to scare anyone. Just inspect any area your going to put your treasured items.

so why is it that a lot of providers offer unprotected oral? is the risk so low that its overlooked?

yes, the risk is really low, so most ppl tend to ignore it.  Furthermore, some providers will also do additional checks to to minimize the risk even more (experienced hobbyist will also pay attention to the signs).  Other reasons I could think of is that:  

1.  Most guys prefer BBBJ.  Don't ask me why guys are not afraid.  We're just that horny
2.  Providers can get better review rating for giving BBBJ.

Just pay attention to signs/symptoms and there will be no problem.  Some of the older hobbyist here can tell you that they've been hobbying for 20+ years and never have a problem.  Stay with well reviewed ladies with good hygiene.  If you're still paranoid, go with covered service

Also, it matters if she allows cum in mouth or not. The risks are lower with no cim.
