I think that in order for a provider to White List you she has to feel safe.

In my case, all of the ladies who have White Listed me did it after just one visit.  I guess I am just lucky that way.  I do my research prior to any encounter.  By the time she crosses my threshold, I am pretty sure that we are going to hit is off and I don't think I can count even on one hand the number of times that the appointment has gone south in almost twenty years of hobbying.  

Maybe it is because I am just a "Big Ol' Teddy Bear" and the ladies just love them some bear.

To specifically answer your question, if you had a good time and the lady appeared to have had the same, then why not ask as she leaves?  All she can say is no.

Just curious what the group thinks - how many times should one be with a provider before it's ok to ask for a white list reference? I'm new to this so I don't want to be inappropriate.

It never hurts to ask. Some will want to see you regularly while others may be OK after a time or two. Ask but put it in a way that she feels comfortable deferring it to a later date, if at all.
