I've wondered too when I've seen a long break in time between reviews.

Question for experienced hobbyists (and providers as well).  I have thought about seeing a certain provider in my area.  She has been in the game for some time, has her own website and lots of (generally very good) reviews, mostly from members who have posted numerous reviews.  I emailed here and she responded right away and was fine to set something up.  

However, I've noted that her last review is more than a year old, whereas there was at least one review a month up until that point.   Could this be a red flag?  It could just be that she took some time off, had a baby, or any other innocent explanation.  Any thoughts?

she has reviews.. you know she's legit.. what would be your concern?  
Could be, she has been under the radar (UTR).. I saw oneproviderwith well over 200 reviews who asked that I not write one up.. she figured she had enough .. so I didn't..

you should be fine.

I would PM several of the last reviewers to be sure that they don't know of any possible compromise in her working for uncle leo if she had been discovered. This is always a possibility, even though a slight one.

She could have taken some time off, been working more and only providing part-time, seeing mostly regulars or those who do not review, or touring areas that do not review on TER. I personally have not had a review since last summer, but that is mainly due to touring areas that do not use TER at all (and hence, do not review on TER) and working more/providing less.

Everyone needs a break.  This is not an easy job.  If a provider takes a year off it could mean anything.  Never assume just ask her directly.
